This video covers the existing published medical research (as at August 2019) on what costochondritis (and Tietze’s Syndrome) is, and how to actually fix it. It is NOT what most doctors and the popular medical sites will tell you - that it’s a “mysterious inflammation” of no known cause that will settle down soon.
That’s why treating it like that usually does not work - it is NOT supported by the existing published peer-reviewed medical research, and is in fact contradicted by it. Yes - this is nuts, and responsible for literally millions of people still in pain after caring treatments that miss the point.

    Hey Steve, this video is still very helpful almost 9 years later. Props to you for still replying to comments almost 9 years later. I was recently diagnosed with Costo after an urgent care visit for shortness of breath. I have asthma as well but my sob never responded to my inhaler like it usually does. Around the same time I did notice my left side of my upper chest by my sternum was a little sore. If I slouch over “ihunch” I also feel like my lungs and ribs are tight and there is some pain. Have you seen many cases where SOB is one of the main symptoms? While I do have some pain and discomfort, it’s not my main symptom. I ordered the backpod a few days ago and hoping it will help free up my ribs so my lungs can take a deep, full breath again. Appreciate any insight you have. Thanks, Jakob

    Hi Steve, I'm 23 years old, I would really appreciate it if you could read this message and help me. I have many questions: 1) Besides pain, one of the symptoms of costochondritis is crepitation in the sternum, I have this at the level of the 2nd, 3rd and 4th ribs. This happens when I stretch or when I do an extension of the spine with external rotation/abduction of the shoulder. I have to do this "gesture" "movement" several times during the day to feel any kind of relief, is it bad ? 2) I have had this pain for almost 3 years (may 2022) due to strength exercises, weighted dips caused this. I read some pdf on your page, it says minimun wait 6 months to do dips again, in your personal experience, is an exercise that should be avoid or do it a lot less ? I also see the video recommendation you put in the booklist. Whatever, I can't do it to prevent a new injury that i will tell you in the next point 3, just i would like to know 3) I had an anterior shoulder dislocation of my left side in november 2022, I did more than 28 physical therapy sessions, I have not had surgery, but my shoulder is relatively stable (obviously i do not feel it 100% quality like my right shoulder, but its very good like 80%), and i do not have another episode of dislocation. Please, what is your opinion about this: What care do you think I should take with my shoulder while treating my costochondritis? and, Can i do exercises to make my shoulders stronger and prevent another dislocation while I use the Backpod daily for my costo?     Hi Steve My name is Ryan, from Toronto Canada. And for 7 years I've been battling costochondritis and tietze syndrome without knowing. I've been in and out of hospitals, visits to my family doctor, countless number of tests, X-ray, CT scan, MRI, ultrasounds, ECG, physiotherapy, you name it. I'm a healthy 40 year old man, and it feels like I'm having a heart attack. Why can't any doctors figure this out? Am I some sort of unicorn ? They say it will get better, and of course it never does. It wasn't till I visited a neurologist, that she mentioned the word costochondritis. This was the first time Ive heard anybody mention this in my 7 years of fighting with this mysterious pain. So I went home and right away I did some research. My symptoms included severe pain in my sternum. Inflammation around my collarbone, causing it to swell up. Along with pain that traveled to my shoulder, the back of my head, and felt inside of the ear. All of this pain is on the right side of my body, and it is extremely debilitating. Ive developed anxiety and high levels of stress, even sneezing has been a nightmare. By the grace of God, I came across your videos on RUclips. Finally!! someone understood my pain. Finally I didn't feel alone in this fight anymore .And finally I found the answers I've been searching for. I ordered the back pod immediately. So far, I'm on day 5, and I've noticed a significant improvement in my ability to move freely without any pain. I haven't had to take any pain medications or anti inflammatories. And Instead of smiling behind the pain like I've had to for so many years, I can now show a genuine smile because now I have hope.     Hi steve! I have costochondritis on my right side of the ribs for about a year. I've just started to use the backpod, used it for the first time yesterday on the right side of the upper spine and the middle and when i woke up this morning, my right hand is tingling a lot whenever i extend my arm. The whole arm is feeling really heavy and there is a numb like sensation in the hand but mostly tingling like pins and needles. Did i do something wrong? 😢

      Sorry, Rebecca - I'm just too swamped. (Unless you're in New Zealand - I do try to be available for Kiwis.) But look - I may have already given it to you, but here's the long wordy PDF on what costochondritis actually is and what we find works best to fix it. Do read it on a computer, not a phone. The PDF covers using the Backpod for costo, and also the other bits that often need dealing to as well. Cheeringly, these can nearly all be done by yourself at home. It takes a bit of time and effort, but it's not that difficult. I know it's long, but you can skim the bits that clearly don't apply to you. However the detail is there if you need it. It does make sense, but it's also up to you to apply it to your own case. It really should give you the guidance to work your way out of costo - usually. Nothing beats good competent hands-on in-person assessment and accurate treatment, but it's such such a lottery finding someone who's actually good at costo. They will be around, but I don't know how to spot them. Where are you in the world? I do know a few here and there.     What about costo that presents at the back of the spine rather than the front? Feels like the rib head is sticking right into the back of the spine and as you described with turning to each side/limited ROM is the same presentation. Have the backpod and planning to start this week!

      Just treat it the same way as you would costo at the front. It's the same core problem - frozen rib joints and tight scarred muscle around the back of the rib cage. With costo, the extra is that the more delicate rib joints on your breastbone are starting to strain, give and get painful. But it's caused by exactly the problem you've almost certainly got - the frozen rib machinery at the back here your ribs join onto your spine. Since you're tight both sides, it's likely that you're also a bit hunched in your thoracic spine - this is sooo common with everyone bending over computers, phones and games. We call it the iHunch. That's okay - it's what we built the Backpod for. Just use it as the instructions say, and also do all the bits in the little straightening-up program which is also in the user guide. There's a bit of strengthening, stretching, home massage and posture as well as the Backpod. You are definitely not alone. Good luck with the work.       Dunno. I just don't know how tight you are, or how irritable your joints are, or how hard you're going on the Backpod, or where you're up to with it. Just use your common sense. If you do too much too soon you'll likely get a bit sore, so ease off until it's okay again. It takes time - you can only stretch joints so much in one go..

    Hi Steve sorry to bother you but what do I do to target c6 and c7, they are the most problematic for me they don’t seem to be responding as well to the pod? I’m down to no pillows, only feel good stretch when using now it’s relaxing. Went to a chiropractor he told me my spine is loose and gave me some stretches to get movement in my back.

      The stretch on the Backpod incorporates a pec minor stretch, just from the position of lying back with your hand behind your head. It's there deliberately - stretching tight pecs is part of freeing up the total costo problem. All that's happened is you're so tight in your pecs that you've pulled one a bit. It's not a biggie - it'll settle. So - start stretching your pecs as well. There are plenty of videos on it; the doorway stretch is fine. If you're that tight, ideally have a sports massage as well - all round your torso, pecs, back, neck, shoulders and arms. It's all part of treating the total costo problem.

    Hi Steve :) I have had sharp pain that affects my breathing like a heart attack and sever stitch feeling right below of ribs near gall bladder - tears show absolutely nothing of course…. 4 years to going into sports doctor to chat about slipping rib. He injected me with cortisone over my rib cartilage around the 9th and 10th as he couldn’t even touch it without being so painful so more trial and error….. but I want to help myself as no one has. If it is potentially Costochondritus cause by irritation of slipping rib is the back pod ok to use?

    Can costochondritis cause shortness of breath ? I had chest wall pain that led me into the ER . That led me to the pulmonary that led me to do a spirometry test . That led them to give me a diagnosis of stage 3 copd emphysema and im 40 ( very rare age to have emphysema at that age) my doctor said my chest wall pain was costochondritis. It never made sense to me how my issue was chest wall pain even tho i was experiencing shortness of breath . How if i had stage 3 emphysema ( 1 step away from needing a lung transplant at 40 ) why was I just giving inhalers . This had been my experiencing for almost over a year thats been driving me crazy . I wondering IF and how much costochondritis can mimic lung function and breathing issues . If costochondritis can have any impact of shortness of breath and the results of a lung function test ? And if so it would be helpful if pulmonary doctor are aware that costochondritis can also impact shortness of breath that needs to be taken into consideration if it can affect a spirometry lung function test . Ive had a undiagnosed rotary cuff pain for years in my shoulder blade so that makes sense to me and my heart has been ruled out as a factor.

      Hi. It can happen. What you’re telling me is that you’re really tight, and probably a bit hunched also. It’s really common, you are not alone, and what you’re describing as things start to loosen is common also. No worries about the numbness - it’s just that you’ll have been tight through the muscles where your nerves are running through your shoulder girdle and down your arm. These are getting stretched when you lie on the Backpod - which is a good thing. The tingling and numbness is just from the nerves being pulled on a bit as they stretch. It's a lot like sleeping funny on your arm and waking up with it numb and tingling for a bit. Talk someone into doing the home sitting massage on you - shown in the Backpod's user guide and also as this video here: ruclips.net/video/9eLUQX03IoE/видео.html Get them to go hard down between your shoulder blades. Do every few days for a few weeks. Ideally, also, go and see a good massage therapist and get them to work all round the chest, shoulder girdle and the muscles down the arms. That’ll loosen the muscles the nerves are running through. As well, you can start stretching the nerves and muscles gently. Lie on your back on a table, knees bent up, with your left shoulder just off the edge of the table. Hold your palm upwards, then bend your hand and fingers back, then drop the straight arm gently down towards the floor (arm out at 90˚ to your body) until you feel a good stretch and even some tingling down the arm. That’s stretching it all. Hold for several seconds, then lift up the arm to take the stretch off, then do again, several times gently. It’s just a stretch. It’ll all disappear when things are loose enough. That you have the tingles/numbness at all does tell me you were pretty tight to begin with. It’ll stretch (and massage) out fine. I’m also planning to use some inti-inflammatory cream also to see if that helps with the burning, I could walk to day with less pain so thank you for the back pod.

      As the instructions say, keep your chin held in a bit when you're on the Backpod. That keeps your neck in a neutral position. If you're already a bit hunched, with your chin poked out, then you neck will already be tight and compressed. If you pole your chin up even more when you're on the Backpod, then you're compressing your neck even more. Do also do the strengthening exercise for the the muscles which oppose this - it's in the Backpod's full user guide. Plus, ideally, the home massage in sitting for your middle back and neck. It's all part of being really tight to start off with. You just need to work into it all gently. It does take time.

      Sorry, I don't know from your description where you mean. The Backpod's for the rib joints, so over the curve of your ribs between your spine and the inside edge of your shoulder blade. You can easily try other places and see how you go - it's only a stretch.

    Steve August, I've had Costochondritis since 2019. Started using the Backpod in January this year, I use it as instructed. The pain I get is on the front right side of my chest pec area. I'm confused because when I use the Backpod it's the left side of my back between shoulder blade and spine that is much more tight and tender. Also when I do the twisting exercise it's much more restricted when I twist to my left side. My question is can I have pain on the front right side of my chest pec area and be tight / frozen on the left side of my back /rib hinges? Kind regards. Harry Turner

      You go lower. The rib cage goes a lot lower than most people think - have a look on Google image. Generally you want to free up the qwhole rib cage anyway, not just the specific bits in it which are most tight. If you're getting sorer lower down, that means the lower ribs around the back of your rib cage will be tight. So sure, target them especially. Do use the Backpod as the instructions in the full 31-page user guide say. Take it all the way up its progression to long, strong, targeted stretches, and also work up and down over the whole area - spine and out to both sides of the spine to stretch the ribs.

I definitely have the ihunch issue and pain in neck + upper chest + upper back and I did a lot of work in these areas but I have a feeling that all of this stems from tight deep neck flexors.     Hi Steve, my problem with Costo started in February when I had a cold, after a few months of pain when doctors only prescribed anti-inflammatory medications and were unable to help me, I started looking for help on the Internet. I bought a roller and a tennis ball which helped me within a few days, after about a month I was able to go to the gym without pain. four weeks ago I got sick and the doctor prescribed me antibiotics for 14 days. After taking the antibiotic for 3 days, the pain returned. in February, when the pain first appeared, I also took antibiotics. Could this have any influence? I have also been using the backpod for two weeks but I still don't see any significant improvement, but I have noticed that since using it I can take a full breath without pain. I often feel no pain at all during the day. I feel it most when I'm sleeping and, for example, when I turn from side to side. Can the inflammation be more difficult to remove because of the antibiotic weakened my immunity? I'm from Poland, sorry for the spelling mistakes Cheers.

      Hi. Costo isn't an infection, so the antibiotics won't directly affect it. The coughing from being sick is what stirs up or triggers costo. Coughing is a surprisingly strong explosive jolt for the whole rib cage. When the rib cage joints around the back are frozen and can't move to absorb some of the shock, then it all hits the more delicate rib joints on your breastbone. That's what triggers or flares the costo - not the antibiotics. The Backpod is helping because you can now take a full breath without pain - that's progress. You're probably just stirred up somewhat from being sick - hope that's settling now. You may need other arts of the problem dealt to as well. As I wrote in the first comment, when I bought the roller and started using it, I quickly got rid of the pain, but I would like to add that I was also taking vitamin D3 in quite high doses of 8,000 units. I stopped taking vitamin D3 after a month. this coincided with the 14 days of antibiotics I mentioned. the situation did not change until I went on holiday 2 weeks ago where I naturally took vitamin D3 in very large amounts. the pain disappeared very quickly and was practically unnoticeable. I came back 2 weeks ago and about 10 days after returning the pain started coming back. I think I need vitamin D3 to get rid of costo completely. what do you think about it?

    Dr Hansen, surgeon in West Virginia has invented a way to tie the ribs so they don't bang on each other. His videos are on RUclips.* Dr Hansen trained Dr Hansen, surgeon in Bend, Oregon to do the surgery. She did surgery on my lower right ribs and it STOPPED my #65 pain! It was minimally invasive and easy recovery. I suffered to the point of trying to kill myself for 17 years from those psycho spasms! Other doctors insisted I had a kidney stone.       Looks good, and a logical way to stabilise slipping ribs rather than cut the slipping bits (usually the costochondral junctions) out. However this is a surgical procedure for slipping ribs, not for costo. Slipping ribs are essentially the costochondral junctions giving repeatedly - like a floppy hinge. With costochondritis, the excessive movement is taking place at the rib joints on the sternum.

      For sure. The difference between them and the Backpod is simply leverage. The cat cow and similar assume that you're already moving pretty well, and just need these sort of exercises to get freer again. However so many people are so frozen in the middle of their thoracic spine that no personal exercise or stretch has the leverage to drag to really frozen joints free - they can be like concrete. That's because any general stretch for the whole spine spreads the stretch over all those vertebrae, so each one gets less specific leverage. With the Backpod, its small cushioned peak is stretching only a few vertebrae at one time, so you can work up to a LOT of leverage on them - which is what they can need. But by all means, add in the cat cow, etc. as well. They'll complement what you're doing on the Backpod, and vice versa.

    Hi thanks for your videos, they are so helpful. I have just purchased mine and started using it but it’s causing me a lot of pain even though I have followed the instructions. I have not been diagnosed with Costochondritis and I’m quite an unusual case. I have Long Covid and had breathing difficulty and chest pain (infection feeling in lower chest) and been housebound with it for 3.5 yrs and it’s getting worse. Constant infections, hypoxia, MCAS, dysautonomia is just some of the things I’ve been diagnosed with and nobody seems to be able to help and I have a severe feeling of suffocation on a daily basis. In March this year I started to have unbearable pain upper/middle back where my lungs are both sides in the soft tissues and either side of my spine. Only thing that brought the inflammation down was steroids which are no use long term. Since then I cannot expand my lungs at the back and I fear I may have scar tissue as the inflammation was severe and over several weeks. I purchased the back pod to see if it will give me any relief to loosen my ribcage as my lungs are so tight they feel as if they’re stuck in glue. I tried the pod for the second time today and after 10 mins the pain is still there. My back and spine are still sore to touch because of the inflammation so will the pod help with this or make it worse?

    Hi all, I've had my backpod for 6 days, I've used it as instructed each day at least once or twice. I think I've seen some improvement, I have good and bad days with my costo, but I have noticed my left thumb is sometimes numb after use. Had anyone else had this? My pains in my chest are usually central or to the left and are normally on/off slight dull ache or icy burn sensation. Has anyone continued to workout whilst experiencing costo whilst getting used to the backpod? I wanted to get people's general view on whether I should exercise. I should note I have been to A&E before and had several scans. A heart related issue was ruled out and my GP had suggested this is costo Thanks

    I've got almost 2 years of pain due to some intense compression in my T5 T6 T7 right in the middle of my back, just ordered this thro Amazon last week, and it is on its way, should arrive next week. Please give me any advice for this specific area of my body, I can stretch my shoulder blades usually with pull ups, but mid T4 to T8 is what I call the impossible to reach area. I'm very hopeful it works because some nights I can't sleep properly from the amount of compression there. Thank you a lot doc! and will definitely give you an update after using it :)

      Hi. Yes - you just can't reach that T5-T7 area satisfactorily by yourself. Usually it's tight in the spinal joints and tight and scarred in the tough muscles over the top of them. Just use the Backpod as the directions say. However do read the 'Warnings and precaution' section on straight or hollowed thoracic spines. These are really uncommon - most people are too much the other way, after hunching over computers and phones. However you've said yours is compressed, and I can't assess that over RUclips comments, of course. In any case, talk someone into doing the home sitting massage on you - shown in the Backpod's user guide and also as this video here: ruclips.net/video/9eLUQX03IoE/видео.html Get them to go hard down between your shoulder blades. Do every few days for a few weeks. That'll help heaps, then the Backpod should more easily get to stretch the tight joints underneath.

      Hi. I don't think it makes any significant difference. Costo is essentially a mechanical tightness and strain problem of the rib cage. It's like having the hand brake jammed on in the car - you fix it by freeing off the tight piece of machinery, not by additives to the petrol.

    What if nothing is working. I’ve been dealing with this for so so many months now, and I literally can not take it anymore. I’ve tried the back pod, I’ve tried exercises, I’ve tried everything. I can’t eat anything because it causes a flare up. I can’t even go to a doctor because I don’t have any insurance. I’m only 19, and the most frustrating part is the I can’t even live life to the fullest. It’s just draining, and I’m tired of it.

      I sympathise, but if you give into it then it's won. Costo is still just a logical rib cage tightness and strain problem - that's all. Working your way through it is usually not particularly difficult. It does take time, effort and attention though. My own costo is completely fixed - I get no pain or restriction, can do anything, never think about it. This is the normal and expected result from treating costo where I've worked in New Zealand physiotherapy for decades. It's just not that difficult. It's a straightforward physio-type strain and tightness problem of the rib cage. It's not a big deal - IF you understand it correctly and treat it correctly. All the doom and gloom about costo is because most countries of the world do not do this. See the PDF in my reply for a route map out of costo. Read it on a computer, not a phone. Yes. it's wordy, and it takes some time and effort. But it's there if you engage with it accurately - that's on you.

I’ve been on the pod now for 3 weeks now, a sports massage and one visit to a chiropractor he said I should come in for 6 treatments as well did the massager.     Hi Steve, I have been suffering from costocondritis for the last 2.5 years. I have been using the backpod for the last two months (not everyday) and almost 80% of my pain is gone but the pain seems to come back when I don't use the backpod for 2 or 3 days. I have reached to the level where I don't need any pillow under me. In my case the costocondritis started due to doing some strenuous exercises like pushups and pull-ups. I have slowly started doing pushups again (8-10). I am also able to do some cardio exercises without any pain but when I do running it starts to hurt, so I stopped running. In my case the pain is mostly in the upper ribs. I think 2nd and 3rd rib. I still hear popping sound when I stretch my chest. Using backpod and doing cardio exercises helps me alot. I wanted to ask you how long it would take for the pain and popping sound to go completely? And thank you for inventing such a great tool. Please provide me with some advice on what should I improve.       Hi. Thank you. Good that you've improved so much. To get the next bit probably needs a few other parts of the total problem dealt to. Here's a long wordy PDF on what costochondritis actually is and what we find works best to fix it. It is more easily read on a computer, not a phone. The PDF covers using the Backpod (or similar) for costo, and also the other bits that often need dealing to as well. Cheeringly, these can nearly all be done by yourself at home. It takes a bit of time and effort, but it's not that difficult. Do see especially Section (2) on taking the Backpod all the way up its progression to long, strong, targeted stretches, plus the sitting twist exercise afterwards to work the joints just that bit freer.
